Looking to create a web form to collect measurements of custom clothing from customers. The form needs to be created in Ninja Forms for wordpress. I have attached a pdf with the fields and measurements we need to collect. Use conditional logic and styles. (These can be put in the custom css part of the theme)
The key is not to copy the pdf layout or style but use the information to create a pretty looking responsive form which is easy to follow and use. You must extract the pics from the pdf
