Create Wordpress plugin

Hi, I need Wordpress plugin which have the same functionality as this calculator: [login to view URL] , and is shown in content page as any normal page but not as a widget. The calculator should calculate how big crane is needed based on weight (VEKT) and length (LENGDE). The layout/look should be like on the URL but with these requirements from the Wordpress Dashboard 1. I want to be able to change color VEKT slider, LENGDE slide and button (Trykk for å beregne kran) 2. Able to change text for button: Trykk for å beregne kran 3. Able to select text color for button text 4. Able to select min, max and step used in VEKT dropdown 5. Able to select min, max and step used in LENGDE dropdown 6. Able to change text after button /Trykk for å beregne kran) have been clicked 7. Able to insert email address for contact form 8. Able to enable and disable form fields and if required (name, email, phone and message box) 9. Able to change text in form submit button 10. Able to change notication text shown to user after form is submitted Function of the calculator After user have selected VEKT and LENGDE and click on the button (Trykk for å beregne kran) the results should show with text in button. Like this : text + results. Also there should be shown a form to contact regarding this results. Form should have name, email, phone and message box. After user have fill out the form and click button, notication should be shown to user
