Form integration with Woo Commerce/Paypal

Looking to have a web form created that automatically links and sends the form data and order info to Paypal and to an email address with a single notification sent to each. Form fields must be verified, button should be an add to cart button. I have attached the order form. Only field one must be completed. All fields must include alphanumeric characters and/or the special characters shown in the attached file.
I will need to know the amount to create the form and I will need the code in embeddable format to input into a Woo Commerce product page after the job is completed and verified to work correctly. The product price is $6 and payable via Paypal or credit card. I will need it setup so I can easily input the Paypal account and email info where needed.
