Need help with a hacked magento checkout page

Read please, my magento site's checkout page seems to be hacked, however i'm unable to find which file being changed. (I've looked all relevant files to the specific addon and payment option and found nothing was changed) the sympton is simple, visa/master card option selected will show a 'form' that should not be there. i need someone who can help address this issue and find the file or w/e that's being changed for me, there's no fix needed as I can fix once issue is found.
