Need a Simple Android app for my Transportation handling work!

Design is ready, I need an Android app. (It's a simple app)
Mainly, feature i need is that, i can control the transportation of the good from Mining place to the ocean port or our storage place.
The function would be an app to fill in the data of the shipment of the material (example: breña, crushed or fine), date of loading, time of loading, photos of the driver's license of the driver, truck license plate, security license, photo of the truck , photo of the union logo, origin and destination.
Once it is filled out and confirmed, it is sent in the cloud to be opened by the reception user in the port or storage yard.
There you would have to fill in the data of: gross weight, net weight and tare plus photo of the scale
Need guidance, if need admin panel or all work can done through one app.
Mainly, All this data is required to download in pdf or excel once the hauling / shipment of material is closed.
