PHP Contact us form and Testimonial Form -- 2

Job Description:
I have html website and 2 pages are using PHP, contact us form and Testimonial Form, Testimonial Form saves data in DB and retrieves it back to same form. For some reason it stopped working when I click on Testimonial Form it throughs errors and same thing on contact us from after clicking submit form I get this error

Additionally, a 403 Forbidden error was encountered while trying to use an ErrorDocument to handle the request.)
I'm not sure if there's issue with files or server configuration issue. If you know how to fix please get in touch.
