Very Basic Website with Authorize.NET processing and Data Capture

We are in need of a team that can create a beautifully designed frontend to a basic website we need up. There will only be one main page and then a secondary page that will provide a "Thank You" confirmation after the person has provided a donation using [login to view URL] to accept the credit card information. We would like you design the home page based on the very limited content we send you. All person to make a donation after clicking a Donate Now button and then we need you to store the person's information in a basic MySQL database file for later retrieval. You or your team should have great eye for design, must know how to code with [login to view URL] to accept credit payments online AND can deliver this project within 72 hours of accepting the bid. This includes working on Saturday and Sunday coming up. Project MUST be online and ready to go by Monday, August 24, 2015. Remember, this is only two or three pages max and can be considered more of Splash or Landing Page more than anything else.
