Wordpress plugin for webform shortcode

I use wordpress and have a custom profile page with "theme my login" plugin. I want to add a form on the profile page with a shortcode. The plugin needs to have a separate css file that I can edit to style my form and succes and error messages. The plugin should have admin options as well to facilitate the settings of the API call (AdminUserName, AdminPassword). See API details here: [login to view URL] The SCDLL name should also be an option in the shortcode. The form needs to register a UserSCUsername through the mentioned wep api via a POST method. If the user did not register a UserSCUsername before, the form inputs are: - switch (Are you a TransActUser: Yes/No) - UserSCUsername entry - submit button Submit will call the "add" action of the API Call and return to the profile page (theme-my-login). If there was an error or success from this call this is displayed on the page as well. If the user already has a UserSCUsername registered and needs to change it the input of this form are: - Old Account name - switch (Are you a TransActUser: Yes/No) - New UserSCUsername - submit button Submit will first call the "remove" action of the API Call and only when that is successful it will call the "add" action with the new UserSCUsername. Return to the profile page (theme-my-login). If there was an error or success from this call this is displayed on the page as well. I am a developer myself, but have other fields of expertise.
