I'm looking for a professional to help me create a repeatable survey aimed at gathering employee opinions. The survey should primarily use a rating scale format. Also, I want to be able to use the survey for 1) Multiple companies 2) Multiple employees per company 3) Plot simple graphics at the individual scores for each survey taker as well as average scores across survey takers Key requirements: - Design a survey that can be deployed as needed, with questions that are structured under primary and secondary levels (4 primary categories and each category has multiple sub-categories) - Each company will have their own unique ID under which employees of that company who take the survey will be grouped - Each survey taker should have their own unique ID for log-in and the results of each survey should be attributable to the survey taker - Scores will be selected as radio buttons by the survey takers - The survey should use a rating scale format (0,1,2,3) - Design a survey that is responsive and mobile-friendly to ensure usability on various devices. Ideal skills and experience for the job include: - Prior experience in designing employee surveys - Understanding of effective survey practices - Proficiency in developing rating scale questions - Ability to create a survey that is engaging and easy to understand for all employees. The survey should include fields to identify the respondents to allow follow-up discussions. The survey should be accessible through a mobile app for convenience. The survey should include interactive dashboards for visualizing results.
