This form needs to appear in specific positions based on the device screen size: desktop at the top of the left-hand column, and tablet/mobile at the bottom of the main content area. This will be achieved by creating two placeholder <div> elements, detecting screen size with JavaScript, and moving the form dynamically from a hidden source container. The deliverables will include functional, tested code on the live site, clear instructions for adding/removing placeholders, and ensuring no duplication of forms.
