Urgent: Web Design Comp within Two Days Starting the Date of Award

We want a website that is both elegant and with great user experience for a dental healthcare company.
We want to design to achieve the following:
1. Guidance. Guide the customers to desired dental service. This means it has a natural CTA's (call to actions) clearly present in the pages.
2. Progressive. The pages should be progressive in story. The website should be like reading a story book where the visitor will naturally move from one page to another.
All details will be given once the project is awarded. We will give you more data as to which websites we think will help guide you.
The website mock up should include:
1. Home Page
2. Inner Pages
3. Booking an Appointment.
Milestones:
1. Initial Design Mock-Ups for Submission - 20% (At least three studies)
2. Feedback and Comments and REvision - 20%
3. Submission of PSD File -60%
