Simple WordPress Plugin skeleton

I require a VERY simple WordPress Plugin to use as a template to carry on developing myself . Easy job - just take the simplest Plugin and modify it! There are two pages, 1. A simple form that has four input fields 2. On posting the form it opens a second page, performs a maths function supplied by me in PHP on those four fields and displays the results, For example: Page 1: $var1="2" $var2="10" $var3="100" $var4="checked" [checkbox] Formula: if($var4=="checked") {$result=$var1*$var2} else {{$result=$var1*$var3} Page 2: echo "Result: ".$result."<br />"; echo "Var1: ".$var1."<br />"; echo "Var2: ".$var2."<br />"; echo "Var3: ".$var3."<br />"; echo "Var4: ".$var4."<br />"; That's it, I will modify it from there: I need the Plugin to be able to be zipped ready with all files to install by others into any WordPress site Ongoing support and tweaks possible at [login to view URL] the future.
