Wordpress website form fix

I currently have 3 website duplicates edited to work the same way as this form:

The form is already created I just need it to function the same:

IF you go to the debt site and press the apply now, it changes the form to “congratulations for submitting…” and the main page changes to a thank you section.

This is what the problem is: On the new sites if you press the “apply now” button just disappears and nothing happens (I don’t get an email about it, and no thank you screen pops up). This is a major issue.
