Custom XMP Panel for Adobe Photoshop

I have to open 5 panels (IPTC-Status IPTC-Image etc.) and input metadata, I want a custom panel that has 17 fields that line up with the other panels so I only have to open one panel and it will fill in the spots in the other panels I want it to match the CS3 custom panel I have uploaded below
