Need a 18+ athletic female model to shoot selfies and / or photos for a new sports cream product

I am looking for a 18+ athletic female model to shoot selfies or have someone take photos of this model in beautiful local locations, where running and athletic activities would be suitable. For example, one or a series of pictures could be a jogging outfit in a local park with our product in her hand or somewhere next to her. Another example could be posing near a beach volleyball court with our product. A third example could be a picture of a model with our product on a running track and field. Iphone, other smartphone, or cameras are acceptable devices. We are looking for at least 15 unique photos and at least 3 unique locations of your choosing. I would need you to sign a photo release. My company will use these photos to promote our product on social media and possibly our website. Lastly, I would need you to fill out the "Permission for Photo Buyer" form with a 18+ person signing as witness and mail it to our headquarters in the header of this document before we can start accepting photos from you. After we receive your form in the mail, we will ship a bottle of our product to the return address so that you can begin your picture shoot.
