Google Alerts modification needed ASAP

Need to set up Google alert that exclude previously given search results. For example, here is the alert that I have set up....

"Brazilian Butt Lift" ~commercial OR "Brazilian Butt Lift" ~cosmetic surgeon

I want to tell Google to

1. EXCLUDE any search results in my Google history. _**AND**_

2. EXCLUDE any URL previously visited in my Google history
