osCommerce - Percentage shipping

Looking for a shipping module that would work off of percentages and flat rates for OScommerce. Please do not bid on this project unless you are confident in your expertise. Quick turn-around.
EXAMPLE:

This is how the information is entered into the cart admin (based on price):
100:24.50,250:37.50,500:49.50,1000:99.50,2000:10%
So, shipping from $1,001 to $2,000 should be 10%
And shipping from $501 to $1,000 should be $99.50
(you get the basic idea)
I'm attaching the base file that only processes flat rates.
