Need Website upgraded from Magento 1.4.0.1 to 1.8.1.x

We have a version 1.4.0.1 Magento website that we need upgraded to 1.8.1.x.
We have a few customizations (in a separate folder) that will need to be added into version 1.8, or maybe you can tell us those features have since been added to the standard version 1.8, so the customizations are no longer needed.
Please list the steps you will take to ensure all changes actually work *before* over-writing our production site.
