Create an algorithm and calculator for life expectancy

I am looking for someone to create me a life expectancy calculator.
The result should be a mathematical formula that I will pass some arguments into and it will give me life expectancy for a person. Written in PHP with a simple form.
The formula will then be used in the mobile app too.
We will be building something similar to this:

The calculation I think could be based on WHO data on average mortality:

You will need to do your own research on how to calculate the value, figure out the most relevant arguments (age, sex, sports, smoking, life attitude, driving a car, country you live in, …) It should not contain more than 10 variables.
