Fix bug in zetaprints web-to-print extension for Magento

We are looking for someone to fix a bug in the zetaprints web-to-print extension for Magento. The problem can be seen on this page [url removed, login to view] . When the user attempts for upload an image (agent photo) the app gets hung up. If you refresh the page and click on "Agent Photo" again you can see what should happen.
