I need a quick job doing which will pass a websites form data through an api to a clients database.

The website is built using a drag and drop landing page builder called Instapage. I do not host the files on my own server, it's completely controlled and hosted with instapage. I just allow my domain to be linked to it.
Instapage do have a solution for sending form data through an api via a webhooks integration. It will pass the data on to an endpoint url. This end point URL would be a script hosted on my server, which will then forward on field data to the database I'm sending too

The visible form fields I need to send over are: first name, surname, post code, telephone, confirm telephone. I do not need all fields on my website sending through the api.
I have included the api spec I have been given, and the url which the script will post the leads to.
It's a small job and needs completing within a few hours from starting.
