Shopify Customizations - custom coding experience required

I have a shopify store that needs some customizations.
Scope:
1) Checkout Page
- At checkout, I would like an add-on product that would provide insurance on any lost packages that are shipped. The accept/decline box would be pre-selected. Pic attached of similar product offering.
2) E-mail Automation
- Once customer purchases a certain group of products, I need an email to be sent to that customer with a formated script along with a PDF attachment.
3) Shipping
- I currently have next day shipping as a shipping option. I need this option to be turned off at 3 PM EST every Friday and resume on Monday morning at 12:01 AM EST.
