Configure Varnish cache for optimal wordpress operation

I have had varnish cache installed on my server and it is working to cache images and other non cookie resources.

I also have wordpress installed as a multisite and because of the way it uses cookies, most of the pages are not cached which is not good for performance.

I also have a file that stores the IP of the user making a request to it via ajax but because varnish is running, every access carries the IP of the server and not the user. I have mod_extract_forwarded installed and running on apache2 but the clients IP is not being forwarded to the file.

I need to have the varnish cache config file ([url removed, login to view]) updated so that the site is running as fast as possible with best settings for a wordpress blog with varnish and also to have the clients IP address forwarded with requests to a particular file on the server.

you must have installed Varnish to a server already and be familiar with it's operation and configuration. I am not interested in someone who copies and pastes a config file from a website. I have already done that and it's not what I want. I need someone who knows why they are adding items to the config.
