simple site with dynamic user content

I need a simple site with 7 pages using a database for dynamic content based on user info:

1) Admin page to log in, add new users and passwords.

2) User page - users log in (if unsuccessful, simple error message displayed)

3) If log-in successful, user comes here, asked 5 questions to which he enters 5 text answers

4) User sees his 5 questions & answers listed with radio buttons so he can select his 1 best answer

5) Here, user sees his top selected answer from 4

6) User sees his 5 questions & answers listed. User asked to enter a number of his choosing in a field next to each answer.

7) Here user sees his 5 answers sorted by number entered in 6

The passwords should be securitycoded

Please note: all 5 answers, as well as the top answer selected on page 4, as well as the numbers assigned to each answer in 6 should be saved so that the next time the user logs in and goes directly to pages 5 or 7, he can see his saved responses.
Also, the user should have the ability to return at any time to page 4 or page 6 at any time and change his selections.
