a simple html form in .net

I am looking for a contact us form to be saved in a mysql database. Below are the form fields:

Name

Email

Phone

Member (Yes/No)

Comments

once the data is saved to a mysql database on the backend, I want this information to be printed on thank you page.

it is just so simple work and i want this in max 1 hour,lower bidder wins the race.
