Form Submission With Results Based on Zip Code using Google Map API

We will need a form set up to collect user information and send an email to the form owner. There are three radio buttons, depending on which one is clicked, will open up fields that were hidden. Please see attached document for form fields.
Pull up closest address
Additionally, depending on the customers zip code, the form will return the nearest location to their zip code. We have an excel list of addresses that will help determine the distance, however we will need to use Google Map API to determine what location is the closest to the customers. When this is calculated, we will return the address that is closest so the customer knows which one to drop off their product to.
We started to build out the first part of the form, however we need you to hook up the 3 radio buttons (hidden fields that appear based on the radio selection) and we will need you to program the location results as well as the email to form owner. Please see attached document for fields and form example.
