PHP / MySQL Client Service request system

I need a very basis system however it must fit in with my website design, so things may have to be in a two column table. Orders are requests to dispatch a cd, manual and receipt. The clients need to be able to see the orders but not modify them once placed. Need some validation for required fields etc and email verification. The system will allow me to enter a price once I have dispatched it. We both will need to see the totals. I need to be able marks orders as Paid / Awaiting Payment / Cancelled and modify the order if needed. Need two other fields on the account record, Affiliate code (which should be automatically generated) and Referring Affiliate code. Affiliates will be awarded a fixed fee (which i need to be able to change somewhere) when the new client has placed their first order. Need a copy order feature for both client and myself. Need some basic user management including forget password feature. I don't need to be able to produce invoices. Need comments through-out the PHP code so I can make modifications. ## Deliverables

1) Complete and fully-functional working site / PHP as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables MUST be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
