WP : Multipart form - repost

We have a data collection form for prospective new clients and we would like it reproduced on our Wordpress site as a multipage form.
The PDF (8 pages) is attached for reference.
All data is to be emailed to a given email address with a copy back to the sender too.
Formatting / colours : to match in with the existing website ([url removed, login to view]) and will have a right sidebar visible
Ideally the form should be a single page using AJAX

I understand the goal of the project. It seems to be a project rather than a continuous work. A can implement the AJAX/JS form with paging within about 8 hours. (Depends on how fancy it should be.) A can also submit Daha Fazla
