Duties and Responsibilities: • To collect data from District, Upazila and Community level and documentation it in a useful format. • To assist in the preparation of a future Health Management Information System (HMIS) in collaboration with HMIS Facilitator-CHTDF-UNDP. • Facilitate the analysis and the interpretation of data by the DMO and CHTDF HMIS-Facilitator • Facilitate the CHTDF HMIS-Facilitator to update and maintain the HMIS on a monthly basis. • Facilitate the DMO in production of tables and health-mappings of District & Upazila • Any other functions, responsibilities or portfolio which may be assigned by the RHDC management. • To provide technical, strategic and administrative support and supervision to the CHTDF Supported health project staff (CHS Field Supervisors, District Store Assistant& CHSWs, etc.) at district, upazila and community level. • To maintain close communication and collaboration with the MOHFW official at district & Upazila level, Cluster Leader-Health and DMO of CHTDF • To liaise with the CHT and GoB institutions, NGOs & all other relevant Stakeholders on a regular basis on the planning, implementation, documentation, monitoring and evaluation of all health activities at the district • To prepare work-plan for the health program • To prepare monthly, quarterly and annual progress reports • To undertake any other assignment given by the HDC Management • Reports to the Civil Surgeon or any other official as designated by HDC (e.g. Health Focal Person), and for purposes of technical and administrative coordination with CHTDF, works closely with Cluster Leader-Health, DMO-CHTDF, PO-CHTDF and District Coordinator.
