Writing an agreement of equipment usage between the company and its photographer

I need a 1-1.5 page agreement between my company and our photographer. The brief idea is as following Agreement of equipment usage Between Company____ Photographer ___ I need a document signed between photography company and it's photographer regarding the equipment usage. The photography company offers the equipment to the photographer to let him finish the shooting for client. The company 100% owns the equipment. The photographer is only the user of these equipment. The photographer must take care of the equipment during the time the equipments are on his hands. If the equipments have the problem caused by manufacture defect, the photographer won't have responsibility to repair it. If the equipments were damaged due to improper usage, the photographer will have the responsibility to cover the repairing cost. These damages including dropping on the floor, spilling the water on the machine, crash the machine on object, broking the parts, getting dirt and. The photographer must hide the equipments in the vehicle truck and covered by dark cloth or plastic bag. After finishing the photography, the photographer must return the equipment to company.
