My proposal would be - A workbook with 3 Sheets - Sheet 1 is visible but completely locked and shows to the students the questions and replies in a "Questionnaire format" No data is stored here. This is a just a sheet that shows what is stored in Sheet 2. This sheet will however have a button next to each item. The button will open a userform. The userform will store the data in Sheet 2 and the data will be then visible in the Sheet 1. - Sheet 2 is hidden - is where the students replies are stored, inside an unpivotted table - Sheet 3 is Hidden - Is where all the items (questions) are stored. Also the various setting will be stored here (i.e. Item 1 is a multi-selection, item 2 can have only one reply etc). This will be the "admin" sheet, if you prefer and I will create dedicated userforms so can be easy to maintain and the admin doesn't have to change the tables in Sheet 3. - Everything will be locked and a special password will be needed to see anything else than what is in the Sheet 1 The password will, of course, be provided :) Warmest regards

Verification string which you can use to detect cheating or modifications to the file. 4. Main page, where the student can launch the test, and see his score after he took the test. 5. Interactive application like dialogs for multiple choice questions, single choice questions, and fill in the blanks. 6. Navigation modules with functionality for going to previous/following question, and to restart test. In addition to the file described above, I would deliver a basic manual with screenshots for usage as teacher and as student.
