Podio Web App Form

Hello, Thanks for taking the time to read this quick job. We use Podio to run various projects, however we need an experienced person to build a form using three questionnaires we have in word format converted into a online link we send out to new leads to fill out answers we need. All questions are structure. 1. Client Questionnaire 2. Marketing Audit. 3. Hypnotic Questionnaire 4. Combine all three (an option to remove all repeat questions) (40 Questions - with some strategic drop down need to be added. ie: Do you use Pay Per Click? (radio buttons) Yes*/No/Don't Know *If Yes Adwords ID [Field] for Adwords ID [222-222-2222] Most most questions will be TEXT FIELD, but will need intuition with the field layout with a combination of Text Contact Category Date Link Image Google Maps Question Number Money Duration Progress slider Calculation App reference.
