button /touch screen interface

I need an interface written in html/javascript for use with tablet pc.I need five buttons on the left for single choice like radio button and on this buttons when you click the background must become red and the button must take a value. I need also five buttons on the right but for multiple choice like checkbox and when you push the button the background must become red. I pay 15$ for this project but if you are interested in this and you do a good work i've a lot of work to give you.
