537247 Membership & Form Feature

Need joomla membership feature to allow users to create account with user name and password capabilities. *Members must be able to manage their account information at any time, also there should be a button for members to cancel their account in the account settings. Need form to instantly tell members that their application has been submitted and will send them welcome message by their email to let them know their membership is accepted for ATM use. Need admin access to manage form fields in which tool tips can pop up to help users complete online form The contact form and all forms should look like [login to view URL], jpeg screen shot is attached. All members will post their ATM receipt information on the website, in order to receive reimbursement on their fees. This is the information needed for the surcharge form: First Name: Last Name: Withdrawal: Checking – Savings last 4 digits of Credit Card (****) ATM Surcharge Fee Amount: ........ Pm (Send Button) There should be a send button within the bottom of the receipt once they send the receipt it sends message out a message on date their surcharge fee will be reimburse Need a forgot password tab where users can reset it if forgot pass word Need conformation that info has been sent
