SIMPLE ASP.NET SAMPLE

I need a simple web site project made in Visual Studio (2005) that makes the same as this: [login to view URL] I will need the same fuctions basically. But in another format design (a simple one, i just care about having it working properly). The project must be done in ASPX with C#. I need it to be written clearly and with notes that describe what is doing each part. I prefer it to be using a MySql database. I need the source files, and the database sql file. This is a simple task for a student homework that is supposed to be very easy for .NET developers.
