Childcare Website with Registration form and SEO

Hello, We are looking for a site design for our daycare. We want the site to have full control via password protected CMS as well as "pre-registration" form. When they click "submit" it will email the information directly to us, save in an SQL Database, and pop up the PDF file with thier answers entered in specific sections of the file for the customer to print, sign and bring in to our office. The site will be required to be done in ASP and will be hosted on a windows platform that we will provide. All design and testing will need to be done on your server, however once complete you will be required to set up the SQL and install all files on our system. The site will need to be fully SEO and we expect to be listed on google and yahoo's first page when searching "Tucson Daycare", "Tucson Preschool" and "Tucson Childcare" within 60 days of the site being live.
