Web Design From Template

I need a website that is very close to [Http://[login to view URL]][1] I also have attached a template and coporate logo that I would like to incorporate in to the design. I would like for the website to be done with cascading style sheets so that I can change the look or feel if needed. I will provide all content for the website. Basically I just want to have the same page layout, navigation system, (buttons, menus, etc...) as [[login to view URL]][1]. So I guess it would sort of be a combination of both the template and the the homebanc website. I do not need all of the funtions that are currently on the homebanc website such as the Calculator link or the spanish link. I will provide a list of the pages that I will require for this site. As for the dynamic parts of this web site.... I would like to have an online application for loans. I would like for this to be simply entered in to a form and stored in a database, either SQL or ACCESS. I would requrie a small admin section to login and dislplay the submitted loans. There would also need to be a dynamic section to add job postings and keep updated, and a link for users to upload resumes in word format. Unfortunately I am under budget constraints on this project so I will consider coders that do not have much or any feedback. In return I will be patient and available 24 hours a day if needed for chat and email, and will provide incredible positive feedback for the coder.
