201414 PHP form+write barcode on pdf

Online form and barcode printing in pdf format. We need to implement an online form to a website. The design would be made by ourself. The user will fill a form where he will enter some personal information to attend a seminar. When the form is filled, the information would be placed on a A4 pdf form document where the logo of the comanpy and some fixed text exists. For each form a unique number will be created (fixed digit) and this number would be written on the pdf document as barcode + text. All the data would be kept on a database (MySQL) and before the seminar the company need to export this data to an excel file where the user name and his registration number should be written. PHP and MySQL database would be used.
