70459 Form+database+graph

I need to create a project which consist of : 1) A sort of form (3 different) where insert some datas 1a) form general info (name, city, http address) + 16 questios yes or no 1b) form with general info + 29 questions to assign a vote from (0 to 3) 1c) general info and only 5 questions to vote (0 to 3) 2) a database in mysql who record dates from point 1 3) a script who read database, elaborate datas (thank's to some mathematical formulas written by me), show result in a sort of table and some graphics.
