Help troubleshooting a php webservice using nusoap(repost)

I need help today debugging a nusoap webservice I'm trying to get running today. I was using another coder who appears to have done a good portion of the work - but is no longer available to complete the project. I suspect there is only a small number of issues that need solving and am looking for a php/apache expert to help. I am using nuSOAP (see attached) to help with my php soap object. I am looking for somebody to work with me in realtime to sort out this problem now, either by looking at the attached code and finding an issue - or logging in to my computer while I access and debug any server settings which may be the problem. I've attached the scripts less my actual server passwords so you can test on your own site. The client, when run, should which should access my nusoap php script but is instead returning an error. For testing purposes, you can use the attached script and search for the email "[login to view URL]" The script should return the city, state, zip in the online mysql database. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
