PHP Ajax CSV SCRIPT - URGENT! WILL PAY WELL!

Convert data record to HTML and Create an Ajax/PHP script to perfom simple write function. URGENT MUST BE DONE IN 3 hrs [login to view URL] PAY WELL to achieve this time! Succesfull bidder will be chosen in the next 1hr. DELIVERABLES 1) Convert [login to view URL] to [login to view URL] format, with the additional combo box and 2 buttons. Should be 1 long HTML page. 2) Using the [login to view URL] structure create an Ajax combo box that allows a user to assign a category to the record. FOR EXAMPLE if the user clicks on "places to eat" Ajax loads LEVEL2 menu list. User selects a LEVEL2 ITEM which loads the LEVEL3 list for that LEVEL2 item. User can write this level 2 category for the record or select a LEVEL3 item, this LEVEL3 item, if clicked is the ONLY item that then remains in the combo box. 3) User clicks WriteToFile. This then writes the record to '[login to view URL]' BUT adds the level structure (see [login to view URL]) AND combines fields StreetNumber, StreetName and StreetType into 1 field Street, in '[login to view URL]' Make sure 1 character space is added between these fields when writing to the 1 field so the StreetNumber and StreetName and Streetype are seperated correctly. Example of written record = '10/304 Smith Street' 4) User can repeat step 3 as many times as they like, changing the category structure for the record, which will write the same record again to [login to view URL] but the new category structure. 5) If the user click FINISH that record is hidden from the table and the user moves on to the next record. I will be on Skype or MSN to answer question immediately one the job has been give to a a bidder. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
