Create a form

I want to build a form to participate in multiple activities of singles and doubles. Player information is posted and includes First and Last name Telephone contact Birth date Unique ID Number which I will assign when they sign up e-mail Home venue (place to play) The form will permit a player to have a ladder ranking everywhere venues participate. To publish the unique player id# for singles and a partnership id# for the specific doubles team. To find specific player information, the directory will get those details by searching to the player id# which will find that player in singles or any partner. Places to play will be solicited to use the ranking system without any fee. Each ladder would be limited to 100 players or teams. I have an account with Form Builder ([login to view URL]) and would like to have it so I could duplicate it on my account. All requirements to accomplish that transfer would be included.
