This document will be used to facilitate a discussion among physicians leading to a consensus statement on the role of family physicians from Asia Pacific countries in managing patients with Pain Management. This resulting work will be aligned with efforts from the World Health Organization and WONCA. Review of current literature, guidelines, and best practices in the following countries should be included and referenced. • Indonesia • Malaysia • Philippines • Singapore • Taiwan • China • Thailand Identification of common approaches would be helpful. A document for review is requested by November 23, 2019. The following text may serve as background: Pain management is a fundamental tool in any medical system, and one that is underutilized in Asia Pacific countries. Reasons for inadequate pain management in this region differ between countries, but some common factors include excessive opioid restrictions, lack of access, social stigma, and poor training and education of physicians. These legal, social, and medical barriers collectively create an environment in which patients suffer unnecessary pain and discomfort, even in the face of terminal illnesses such as cancer. In Asian Pacific countries, pain management is generally administered only by anesthesiologists, oncologists, and other specialists working in hospitals. Restricting pain treatment to hospitals and patients who have access to them ignores the majority of the population living with untreated pain. Increasing access to hospitals and specialists alone is not sufficient action to address the undertreatment of pain and would overburden these institutions. Educating primary care physicians in responsible pain management is necessary in order to reach these patient populations and provide them with both short and long-term treatment. Family medicine physicians are uniquely positioned to educate and treat their patients in pain management and dispel stigma in their communities. A dialog is needed to create the infrastructure necessary to support responsible pain management in Asia Pacific countries so that pain medication is neither under nor over prescribed.
