Create MySQL Database and Develop application using C# and WPF

I have a project to develop MySQL database and software using Visual Basic 2013. Project Tasks: • Create MySQL database with table names columns as defined in attached files • Upload data to database tables as follows: o Zip Codes to the zip codes table o City Names to city names table o State KML data to the State KML data (each state must between database and excel file) o NY Thru miles to NY Thruway miles (each tab has x and y points for corresponding to database table) • After developing mysql database use word file (attached) to develop application forms in visual basic 2013 (VB.Net). The forms would not have any function for now. All of the buttons on screens in the file need to be in the Microsoft Ribbon and rest of the form needs to be developed in the way that it can be used to enter data into sql database. • For the application I need to use the same interface as Microsoft Word 2013, same color, same font, same theme, same ribbon. Use the latest trend in user interface but make it similar to Microsoft Word 2013 as much as possible. I need someone who can develop fast as this project is large and I will have more similar tasks for this project. Keep in mind when posting you bid that if job is well done there will be more work on the project. Miles stones as follows: 20% when project is awarded 30% for MySQL database 25% for Visual Basic application forms 25% when project is tested and confirm to be functional as described in project tasks.
