For this amount, I could: 1.) Convert the PDF to ASP.NET page. 2.) Setup the ASP.NET page to post to MSSQL Server (I assume you want to store the data somewhere). 3.) Setup MSSQL to email the results to whomever. But as far as "re populating" the PDF form, that's another animal However, you can get someone to add that on a separate job. They will go hand in hand.
