Easy form for someone who knows

Hi everyone, what I need is this: Create an HTML form with the following fields: 1. Name (text field) 2. Email (text field) (Make sure you add validation to check if its valid email as well) 3. Coder or buyer (drop down: "Coder" or "Buyer". 4. If the user selects "Coder" from the drop down then a field should appear (or be enabled) that says: "URL" which asks the user to enter a URL. This is just a simple text field. Also, another field should appear or be enabled that says: "Project type:" and the options available are: "Big project", "Medium size project" and "Small project". 5. However, if the user selects "Buyer" from the drop down, then a field called "Attachment (Zip file only)" should appear or make enabled. It will be a simple text field with a "Browse" button on the right side which will let user browse for a zip file on their computer. Please make sure not to allow user to select any other file format other than .zip format. 6. There should be one last field called: "Comments" which should be a text area where users can enter as much text as they want. Now, the required fields should be only the "Name", "Email", "Coder or buyer", "URL" and "Attachment". The optional fields will be the "Project type" and "Comments". When submitting, it should send me an email with all the information the user submitted. So please make sure that if the user selected "Coder" then everything should be sent including "URL" and project type. If user selected "BUyer" everything should be sent including the zip file as an attachment. It should also insert a record into a simple MYSQL table on my server which you also need to create. Just provide me the script so that I can create the table myself on my server. Insert just the name, email and the IP address of the person who submitted the form. It can be IP address or some other unique number that can identify the computer which submitted the form so that user should not be allowed to submit the form more than once. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform HTML FORM
