Online database for online staff

Hi. I need and online database that can record simple information about potential or existing part time workers. 1. New applicants should be able to register their details such as name, surname, phone number, email address ,age and postcode, and position they apply for. 2. Once registered and "available" worker should be able to set his profile to "available" or "unavailable" so that we will know if information about upcoming work should be sent to him/her. 3. The part time worker can then accept or decline the work. 4. I should be able to send mass messages to applicants which are "available" either by text, email or both. When the part time worker is "unavailable" he/she will not get a message. 5.I would like to be able to see if a person has registered before with different credentials. 6. I would like the option to "black list" an applicant if they have not showed up for work or any other reason. 7. The database should look the same as my current website. 8. I may need to ask a few qualifying questions before the registration , depending what position the person is applying for. Qualifying question may be " Do you have a current UK drivers licence?"or "Do you live in the Middx area?" 9.I will need a back office function to add questions or change/delete questions. 10. The main function of this database will be: a) Applicants do do their own information input. b) We qualify applicants with a few questions. c) We can send out a communication to all applicants that are available. d) We need confirmation when a person is available for the job. These jobs have very few criteria and will not be complex. Please let me know a time frame and cost but PLEASE don't waste my time if you have not read the full job description. Thank you.
