I need 5 custom forms built - open to bidding

I have a new theme I will be installing on a few different sites. I need to have the application form built out. the form has to be programmed to post to different places and have different fields per page. I have instructions for all of the forms that I need. They all have to have the exact style because the form itself is hard coded in the theme. You can see the form here as it is designed. [login to view URL] The forms all use different posting styles. Some use standard php post others use xml ajax and javascript. they all need to have special fields that will populate. I already have a .js of all the info but I just cant seem to figure this out on my own. We can discuss pricing and details just email me with any questions.
