Advanced Form in Joomla

Hello, I need to create an advanced form using in a Joomla site. The site's address: is [login to view URL] and it's in Hebrew. Just for your info, it’s a restaurant that provides a large amount of food for costumers. Basically, the form's purpose is to allow costumers order dishes using the website. The payment will take place in the restaurant, so there is no payment involved in this work. The form should be made with 3 stages: 1. Selecting dishes from the menu ("Order" button will send the user to stage 2). 2. Getting into a page that tells you your order details and overall price and then asks you to confirm your order. After confirming, the costumer goes on to stage 3. 3. "Thank you" page + a copy of the order to both costumer and the restaurant. There are a few Categories and each one of them contains items. For example: Main dishes: Rice, Pasta, Chicken In addition to selecting the dish, the restaurant needs to know a 2 more things: A. Packing – which packing does a costumer want for each dish he selects (There are 6 different kinds of packages for each dish) B. Quantity – how many dishes per item. Each dish that is selected MUST contain paragraphs A+B so for instance if a costumer picks 2 dishes from the same Category (let's say Rice + Pasta) he needs to pick a Package and a Quantity for EACH ON OF THEM. You have a free hand in how you choose to build this form as long as it answers the demands. The menu is on a CSV Excel sheet in case you want to use SQL. There are about 80 Items in the menu.
