make multiple CJuiSliders work on a single form

I have a form with two different CJuiSliders. However, I am having trouble making them work independently of one another. When I click on the handle on one slider, the value label over the other handle disappears. In addition, the default value label only shows up on the second slider's handle. It is obvious that there is an issue with variable names being shared, but I haven't had any luck identifying the variables that require unique names. The php form is attached.
